Teen participation requires parent or legal guardian consent.
After a parent or legal guardian contacts the Program Director and the request is reviewed, approved families will receive the current consent, safety, and emergency forms through a private process.
How the teen consent process works
Parent or guardian makes contact
A parent or legal guardian contacts the Program Director using their own current phone number and email address.
The parent/guardian request is reviewed
G.O.T. Orchards discusses the program, schedule, eligibility, accessibility support, and next steps with the family.
Current forms are sent privately
G.O.T. Orchards sends approved families the current consent, safety, and emergency forms with private return instructions.
Authorization is verified
The required form must be returned with a notarized parent or legal guardian signature before the teen attends.
What the parent or legal guardian must provide
Approved forms will request the parent or legal guardian’s name, relationship to the teen, current phone number, email address, signature, and date. A teen’s personal phone number does not replace the parent or legal guardian’s contact information.
What the private packet covers
Required private forms include primary parent or legal guardian information, a named backup emergency contact, essential health and allergy information, attendance authorization, dismissal instructions, and emergency medical consent.
A backup emergency contact with a current phone number, email address, and home address is required in case the primary parent or legal guardian cannot be reached.
The emergency authorization covers calling 911, emergency medical assessment and treatment, ambulance transfer to an appropriate local hospital, and lawful use of an epinephrine auto-injector by a trained and authorized person when indicated. All submitted documents are reviewed by the G.O.T. Orchards Program Lead for completeness before participation is approved.
Forms remain private
The complete consent, safety, emergency, health, accessibility, and release documents are not posted for public completion. G.O.T. Orchards provides the current role-appropriate forms and secure return instructions after the parent or guardian request is reviewed.
